Restore unfiltered scanning for pending IRK peers when the controller
cannot resolve private addresses. Keep accept-list filtering while
suspended and when forced by periodic advertising synchronization.

hci_le_set_privacy_mode_sync() needs the IRK returned by
hci_find_irk_by_addr() to remain allocated until its identity address
and type have been copied into the command. The finder drops its RCU
read lock before returning this borrowed pointer, leaving the later
field reads unprotected.
When a privacy-capable controller has a pending peer with address
resolution and device privacy enabled, accept-list programming can
reach this helper while management reloads the IRKs. The request
worker holds hdev->req_lock, whereas load_irks() takes hdev->lock,
allowing the following ordering:
Request worker Management / RCU reclaim
hci_le_set_privacy_mode_sync()
hci_find_irk_by_addr()
rcu_read_lock()
find matching IRK
rcu_read_unlock()
return borrowed IRK
load_irks()
hci_dev_lock(hdev)
hci_smp_irks_clear()
list_del_rcu()
kfree_rcu()
hci_dev_unlock(hdev)
RCU grace period and IRK free
read irk->addr_type
copy irk->bdaddr
The request then accesses a freed IRK. KASAN reports a one-byte
use-after-free read of addr_type on this path.
Keep an outer RCU read-side critical section across the lookup and
both identity-field copies. The finder's nested unlock then leaves
the IRK protected until the caller finishes copying it. Release RCU
on a lookup miss and after copying the fields, before the synchronous
HCI command can sleep.

An ACTIVE devcoredump's buffer cursors must remain valid from the
packet handler's state check through its copy. hci_devcd_rx() holds
hdev->lock across that interval, and hci_devcd_reset() requires the
same lock. However, hci_dev_open_sync() invokes reset without taking
it.
A registered VHCI controller can collect a dump through its debugfs
interface while the controller is down. If a successful open overlaps
with handling a nonempty dump packet, reset can run after the receiver's
ACTIVE check and before its copy:
hci_devcd_rx() hci_dev_open_sync()
dequeue nonempty skb
hci_dev_lock(hdev)
handler observes ACTIVE
hdev->open(hdev) succeeds
hci_devcd_reset(hdev)
dump.tail = NULL
dump.state = IDLE
hci_devcd_copy()
compare against old dump.end
memcpy(NULL, ..., skb->len)
hci_dev_unlock(hdev)
Reset leaves dump.end pointing at the old buffer and purges only queued
packets, so it cannot discard the skb already dequeued by the receiver.
The bounds check can pass with a NULL tail, leading to a nonempty copy
to address zero. KASAN reported a 24-byte NULL write in hci_devcd_rx().
Take hdev->lock around the open-time reset. The receiver then finishes
its state check and copy using valid cursors before reset, or observes
IDLE after reset and skips the copy.

When the controller supports a single advertising set, instance 1 uses
handle 0x00 which is the same handle used by instance 0 (Global
Advertising), but hci_cc_le_set_ext_adv_enable would lookup the instance
using the handle as instance identifier, marking instance 0 as
enabled/disabled instead of instance 1, leaving its state out of sync.
Fix this by matching instances by handle and, for handle 0x00, using
hdev->cur_adv_instance, now set when enabling an instance, to determine
which instance owns the set. Also disable the instance sharing the
advertising set of instance 0 before overriding it.

Global Advertising, enabled with MGMT_OP_SET_ADVERTISING, uses instance
0 and with extended advertising it has its own advertising set, but
instances were still being treated as if they shared the same set:
- Add Advertising and Add Extended Advertising Data would return
success without ever enabling the instance.
- Removing the last instance would disable all advertising sets,
stopping Global Advertising while leaving its setting enabled.
- Disabling Global Advertising would disable all sets, including
active instances.
Fix this by only overriding instances that share the advertising set of
Global Advertising, and by tracking HCI_LE_ADV_0 rather than HCI_LE_ADV
to determine the state of Global Advertising.

Mesh uses millisecond time units for advertisements instead of seconds
as normally. These values go to hdev->adv_instance_timeout, which then
has mixed units. hdev->adv_instance_timeout is however only used as a
true/false flag.
Make hdev->adv_instance_timeout boolean flag, to avoid possible future
problems with its mixed-units values.

Mesh adv instance timeout and duration are in milliseconds, see
mesh_send_sync, hci_add_adv_instance. hci_schedule_adv_instance_sync()
and hci_enable_ext_advertising_sync() ignore this, and scale the timeout
wrong.
Fix for mesh instances by using msecs_to_jiffies() in
hci_schedule_adv_instance_sync and fixing the scaling in
hci_enable_ext_advertising_sync.

During PA sync scan setup, hci_le_set_ext_scan_param_sync() gets the
address used for the PA_LINK lookup from hci_sent_cmd_data(). The
returned pointer borrows storage from sent_cmd or req_skb, without
holding a reference to either skb.
The scan worker runs on req_workqueue while hci_cmd_work() runs on the
separate device workqueue. After the scan worker loads sent_cmd, the
command worker can free and replace it before hci_sent_cmd_data()
dereferences the skb. The command payload can also be freed between
returning from the helper and comparing the address. The connection
lookup's RCU critical section does not protect the command skb.

Use hdev->lock to serialize the address snapshot with sent_cmd

RCU-protected connection lookup. Release the mutex before sending or

The sync command worker holds hdev->req_lock, but inquiry-cache updates
and flushes use hdev->lock. Both hci_acl_create_conn_sync() and
hci_stop_discovery_sync() look up entries and read their fields without
taking hdev->lock.
After either lookup returns, a concurrent HCIINQUIRY ioctl can acquire
hdev->lock and flush the cache, freeing the entry. The worker then reads
the freed entry while preparing a create-connection or remote-name-cancel
command. The list traversal also races with cache updates and removal.

hci_unregister_dev() disables cmd_work and cmd_timer, then calls
hci_cmd_sync_clear(), whose cancel_work_sync() waits for
hci_cmd_sync_work() to return. That worker dequeues and runs every
entry on cmd_sync_work_list. With cmd_work disabled nothing reaches
the controller, so each queued HCI command waits the full
HCI_CMD_TIMEOUT (2s).
The backlog has no bound. Userspace can keep queuing MGMT commands
such as MGMT_OP_GET_CLOCK_INFO against a controller that doesn't
answer. Closing /dev/vhci then blocks in vhci_release() for
backlog * 2s:
INFO: task syz-executor:5749 blocked for more than 143 seconds.
cancel_work_sync
hci_cmd_sync_clear
hci_unregister_dev
vhci_release
In a local reproduction the backlog held more than 5000 entries,
which comes to hours of hang.
Stop the worker from taking new entries once HCI_UNREGISTER is set,
and wake any request still waiting with -ENODEV before cancelling
the work. The entries left over are destroyed with -ECANCELED by the
existing sweep in hci_cmd_sync_clear(). They stay on the list until
the work has stopped, so hci_cmd_sync_dequeue() and friends still see
them. A callback already running may still issue another command,
which delays unregister by at most one timeout per remaining command,
not by the whole backlog.

hci_dev_close_sync() clears hdev->local_codecs after releasing hdev->lock.
Codec list additions and both traversals in sco_sock_getsockopt() use that
lock, but the close path does not. A close and BT_CODEC query can therefore
interleave as follows:
hci_dev_close_sync() sco_sock_getsockopt()
hci_dev_lock()
fetch codec entry
hci_codec_list_clear()

This is another run of the Coccinelle script for converting kmalloc()
family of allocations to kmalloc_obj() via the existing rules in
scripts/coccinelle/api/kmalloc_objs.cocci
This catches both the set of kmalloc() uses added since the first
kmalloc_obj() conversions in v7.0 and adds a large group missed in the
first pass due to Coccinelle not interacting well with the cleanup.h
scoped_...() family of macros[1]. I worked around this with spatch's
"--macro-file" argument to a file with all the scoped_...() macros mapped
to Coccinelle's YACFE_ITERATOR[2] as that was the closest viable control
flow indicator I could find.
Build tested allmodconfig on x86, arm64, arm, loongarch, mips, powerpc,
riscv, and s390 with no new warnings.

create_le_conn_complete() decides whether the failed connection is
still pending by comparing it against hci_lookup_le_connect(), which
returns the first LE connection in BT_CONNECT. That is the same
connection only while at most one is pending.
Two can be pending. Connections created on the passive scan path sit
in BT_CONNECT with HCI_CONN_SCANNING set and are invisible to
hci_lookup_le_connect() until hci_le_create_conn_sync() clears the
flag when their command is issued, so the -EBUSY guard in
hci_connect_le() does not prevent a second connection from being
queued while the first is still on the scan path. Whenever two
connections are in BT_CONNECT at once, the lookup may return one
connection while create_le_conn_complete() is reporting the failure
of the other; the early exit then drops the error and hci_conn_failed()
never runs on the connection that failed.
The controller also rejects a second HCI_OP_LE_CREATE_CONN issued
while another connection creation is still outstanding, per Core Spec
Vol 4, Part E. The spec calls for Command Disallowed there; the
bcm43438 observed here answers with an LMP/LL error code instead,
which bt_to_errno() maps to the -EPROTO (-71) in the log below.
The leaked connection stays in BT_CONNECT forever, and because
hci_connect_le() refuses to dial while hci_lookup_le_connect() finds
anything, every subsequent attempt to reach any peer fails with
-EBUSY and no command reaches the controller at all.
Seen on a bcm43438 with two BLE peers polled on the same interval
(state 5 is BT_CONNECT; both handles are UNSET ones, allocated from
the ida above HCI_CONN_HANDLE_MAX):
Bluetooth: hci1: Opcode 0x2013 failed: -71
# hcitool con
< LE 14:9C:EF:03:68:81 handle 3840 state 5 lm CENTRAL
< LE C4:D3:6A:8C:B5:38 handle 3841 state 5 lm CENTRAL
A btmon capture across the next ten minutes of connect attempts
contains no HCI_OP_LE_CREATE_CONN at all; outgoing LE connections
do not recover until the adapter is reset. With this change the same
scenario fails the rejected connection cleanly and further connects
to both peers go through.
Ask about the connection itself instead of about the device.

A synchronous HCI command that never receives a response leaves
HCI_CMD_PENDING set: hci_req_cmd_complete() is the only place that clears
it, and it only runs when a response matching the last command sent
arrives.
hci_send_cmd_sync() populates hdev->req_skb only when the flag transitions
from clear to set, while hci_dev_open_sync() and hci_dev_close_sync() drop
req_skb without clearing the flag. After a timeout followed by either, the
two disagree: the flag claims a request is outstanding while req_skb is
NULL. Subsequent synchronous commands are then sent with no req_skb, so
hci_event_packet() has nothing to match an arriving event against, and the
caller times out even though the controller answered.
Commands answered by Command Complete recover on their own, since
hci_req_cmd_complete() clears the flag as a side effect. Drivers using
__hci_cmd_sync_ev() with a custom event do not, because a vendor event
never reaches that path. On a WCN3988 (hci_qca over UART) this makes a
controller firmware hang unrecoverable: the driver injects a hardware
error and re-runs qca_setup(), qca_read_soc_version() waits for
HCI_EV_VENDOR, the reply arrives within 4 ms and is discarded, and every
retry fails the same way. The adapter is left down until the driver is
unbound and rebound, or power is removed.
Clear the flag wherever the last request is dropped, restoring the
invariant that req_skb is non-NULL exactly when HCI_CMD_PENDING is set.
Verified on hardware by forcing a command timeout: without this change
setup fails on every attempt, with it setup succeeds on the first.

adv_timeout_expire() hands a kmalloc()ed instance byte to
hci_cmd_sync_queue() with a NULL destroy callback, and only
adv_timeout_expire_sync() frees it. That leaks on two paths:
- the return value is not checked, and hci_cmd_sync_queue() does not
take ownership when it fails (-ENETDOWN, -ENODEV, -ENOMEM);
- a cancelled entry is not released, as _hci_cmd_sync_cancel_entry()
does not free entry->data when there is no destroy callback.
hci_cmd_sync_clear() cancels every pending entry when the controller
is unregistered.

The hci_sync conversion moved class-of-device and EIR generation from an
HCI request built under hdev->lock to asynchronous command sync work.
The worker holds hdev->req_lock, but that lock does not serialize access
to hdev->uuids against add_uuid() and remove_uuid(), which update the
list under hdev->lock.
The following interleaving can therefore occur:

hci_abort_conn() read hci_skb_event(hdev->sent_cmd) when a connection
was pending, but hdev->sent_cmd can be NULL while req_status is still
HCI_REQ_PEND, leading to a NULL pointer dereference and a general
protection fault from the hci_rx_work() receive path.
Instead of inspecting hdev->sent_cmd, track the in-flight create
connection command with a new per-connection HCI_CONN_CREATE flag and
route all cancellation through hci_cancel_connect_sync(), which
dispatches to a dedicated per-type cancel function. The create command
is in exactly one of two states: still queued, or in flight. The cancel
function holds cmd_sync_work_lock across the whole decision: the worker
takes this lock to dequeue every entry, so while it is held a queued
command cannot start running and an in-flight command cannot complete
and let the next command become pending. This keeps the flag test and
hci_cmd_sync_cancel() atomic with respect to the worker, so a queued
command is simply dequeued, and an in-flight command owned by this
connection is cancelled without the risk of cancelling an unrelated
command that became pending in the meantime. CIS uses the same flag
mechanism via HCI_CONN_CREATE_CIS but cannot be dequeued per-connection.
hci_acl_create_conn_sync() and hci_le_create_conn_sync() clear
HCI_CONN_CREATE after the create command completes, but the command
status handler can free conn via hci_conn_del() (for example when the
controller rejects the connection) while the worker is still blocked on
the connection complete event. Hold a reference on conn across the
create command so the flag can be cleared without a use-after-free.

hci_le_create_cis_sync() dereferences conn->conn_timeout after releasing
both rcu_read_lock() and hci_dev_lock(hdev). The conn pointer was
obtained from an RCU-protected iteration over hdev->conn_hash.list and
is not valid once these locks are dropped. A concurrent disconnect can
free the hci_conn between the unlock and the dereference, causing a
use-after-free read.
The cancellation mechanism in hci_conn_del() cannot prevent this because
hci_le_create_cis_pending() queues hci_create_cis_sync with data=NULL:
hci_cmd_sync_queue(hdev, hci_create_cis_sync, NULL, NULL);
While hci_conn_del() dequeues with data=conn:
hci_cmd_sync_dequeue(hdev, NULL, conn, NULL);
Since NULL != conn, the lookup in _hci_cmd_sync_lookup_entry() never
matches, and the pending work item is not cancelled.
Fix this by saving conn->conn_timeout into a local variable while the
locks are still held, so the stale conn pointer is never dereferenced
after unlock.
This is the same class of bug as the one fixed by commit 035c25007c9e
("Bluetooth: hci_sync: Fix UAF on le_read_features_complete") which
addressed the identical pattern in a different function.
This vulnerability was identified using 0sec.ai, an open-source
automated security auditing platform (https://github.com/0sec-labs).

HCI_EVT_LE_ALL_REMOTE_FEATURES_COMPLETE
This fixes not setting the bit for HCI_EVT_LE_ALL_REMOTE_FEATURES_COMPLETE
when extended features bit is set otherwise the controller may not
generate HCI_EVT_LE_ALL_REMOTE_FEATURES_COMPLETE causing
hci_le_read_all_remote_features_sync to timeout waiting for it.
Also remove dead code.

hci_le_big_create_sync() uses DEFINE_FLEX to allocate a
struct hci_cp_le_big_create_sync on the stack with room for 0x11 (17)
BIS entries. However, conn->num_bis can hold up to HCI_MAX_ISO_BIS (31)
entries — validated against ISO_MAX_NUM_BIS (0x1f) in the caller
hci_conn_big_create_sync(). When conn->num_bis is between 18 and 31,
the memcpy that copies conn->bis into cp->bis writes up to 14 bytes
past the stack buffer, corrupting adjacent stack memory.
This is trivially reproducible: binding an ISO socket with
bc_num_bis = ISO_MAX_NUM_BIS (31) and calling listen() will
eventually trigger hci_le_big_create_sync() from the HCI command
sync worker, causing a KASAN-detectable stack-out-of-bounds write:
BUG: KASAN: stack-out-of-bounds in hci_le_big_create_sync+0x256/0x3b0
Write of size 31 at addr ffffc90000487b48 by task kworker/u9:0/71
Fix this by changing the DEFINE_FLEX count from the incorrect 0x11 to
HCI_MAX_ISO_BIS, which matches the maximum number of BIS entries that
conn->bis can actually carry.

hci_cmd_sync_run() may run the work immediately if called from existing
sync work (otherwise it queues a new sync work). In this case it fails
to call the destroy() function.
On immediate run, make it behave same way as if item was queued
successfully: call destroy, and return 0.
The only callsite is hci_abort_conn() via hci_cmd_sync_run_once(), and
this changes its return value. However, its return value is not used
except as the return value for hci_disconnect(), and nothing uses the
return value of hci_disconnect(). Hence there should be no behavior
change anywhere.

__hci_cmd_sync_sk() sets hdev->req_status under hdev->req_lock:
hdev->req_status = HCI_REQ_PEND;
However, several other functions read or write hdev->req_status without
holding any lock:
- hci_send_cmd_sync() reads req_status in hci_cmd_work (workqueue)
- hci_cmd_sync_complete() reads/writes from HCI event completion
- hci_cmd_sync_cancel() / hci_cmd_sync_cancel_sync() read/write
- hci_abort_conn() reads in connection abort path
Since __hci_cmd_sync_sk() runs on hdev->req_workqueue while
hci_send_cmd_sync() runs on hdev->workqueue, these are different
workqueues that can execute concurrently on different CPUs. The plain
C accesses constitute a data race.
Add READ_ONCE()/WRITE_ONCE() annotations on all concurrent accesses
to hdev->req_status to prevent potential compiler optimizations that
could affect correctness (e.g., load fusing in the wait_event
condition or store reordering).
