Pull SCSI updates from James Bottomley:
"Small number of driver updates (mpi3mr, ufs, scsi_debug).
By far the biggest core change (at 4000+ lines) is the sense code
rework to make our sense handling follow the 16 bit defines in the
SCSI standard rather than our split ASC and ASCQ handling which ends
up being confusing and difficult to grep.
The changes should all be functionally equivalent and so far the bots
and AI seem to agree; the code has been incubating in -next since 16
September with one reported issue fixed.

Damien Le Moal <dlemoal@kernel.org> says:
This patch series defines as macros all ASC/ASCQ combinations present in
https://www.t10.org/lists/asc-num.txt and converts the SCSI core code and
SCSI low level drivers to use these macros instead of hard-coded
ASC/ASCQs. To do this, struct scsi_sense_hdr and struct scsi_failure are
modified to replace the asc and ascq 8-bits fields with a 16-bits
sense_code field.
This cleans up the code in many places and makes it easier, and self
documented, to test sense codes in the case of errors.
Overall, there should be no functional chnages here.
Note: this series currently applies cleanly only to the scsi-staging tree.
It does not apply to Linus tree as commit 6d81700ad7c4 ("ata: libata-scsi:
do not raise UA for storage element depopulation and restoration") creates
a conflict with patch 37. The conflict is trivial to resolve, but this
will create a conflict in linux-next. Could you maybe rebase scsi-staging
on rc2 ?
Also please note that sashiko did comment about several pre-existing
issues for different drivers, but I am not addressing these issues in
this series.

Refactor the USB mass storage driver to replace all hard-coded additional
sense codes and additional sense code qualifiers with the enum values
defined in include/scsi/scsi_sense.h. This helps with code clarity as the
sense codes being processed are easier to test and self-documented.

sierra_get_swoc_info() requests sizeof(struct swoc_info) (60) bytes
from the device via usb_control_msg(), but its callers only treat a
negative return value as failure. A device that answers the
vendor-specific GetSwocInfo request with a short IN transfer is
therefore accepted, leaving the tail of the freshly allocated
(kmalloc(), non-zeroing) swoc_info buffer uninitialized.
truinst_show() subsequently prints swocInfo->rev, swocInfo->LinuxSKU
and swocInfo->LinuxVer from that buffer into the world-readable
(0444) "truinst" sysfs attribute. An emulated/malicious USB device
(VID 0x1199, PID 0x0fff) can exploit this to disclose up to 5 bytes
of stale kernel heap memory (kmalloc-64) to unprivileged userspace,
once per sysfs read, indefinitely. On kernels built without
init_on_alloc this leaks recently freed heap contents.
Only accept the transfer when the full structure was received.
sierra_ms_init() already retries failed queries, so well-behaved
devices are unaffected.

ene_ub6250_probe() sets us->transport but never sets
us->transport_reset. get_transport() only fills in transport_reset
for the standard USB_PR_CB/CBI/BULK protocols; since this driver
matches on VID/PID alone, a device can report any bInterfaceProtocol
value and still bind, leaving transport_reset NULL.
When the transport then reports an error, usb_stor_invoke_transport()
calls us->transport_reset(us) in its Handle_Errors path, dereferencing
a NULL function pointer:
BUG: kernel NULL pointer dereference, address: 0000000000000000

kthread+0x38b/0x480 kernel/kthread.c:436
Set transport_reset to usb_stor_Bulk_reset: ene_send_scsi_cmd()
already frames its command/status packets with the shared
struct bulk_cb_wrap/bulk_cs_wrap from include/linux/usb/storage.h
and only ever transfers over us->send_bulk_pipe/recv_bulk_pipe, i.e.
this device speaks the Bulk-Only Transport wire protocol underneath
its own SCSI translation layer, so the Bulk-Only reset is the correct
recovery action for it, matching the pattern already used by the
other subdrivers with a custom transport function (alauda, datafab,
jumpshot, karma).
Also harden usb_stor_probe2()'s existing sanity check to require
transport_reset alongside transport and proto_handler, so a future
subdriver making the same mistake fails to probe instead of crashing
on the first error recovery.

MEDIA_INFO(us).lba_to_pba and .pba_to_lba are arrays of num_zones
pointers, allocated by kcalloc() in alauda_init_media().
alauda_ensure_map_for_zone() reads lba_to_pba[zone] and
pba_to_lba[zone], and when either is NULL alauda_read_map() writes a
pointer back to both. Two separate errors let zone reach num_zones, one
element past the end.
max_lba is capacity >> (blockshift + pageshift), which counts physical
blocks, but zone is lba / uzonesize and uzonesize is 125/128 of
zonesize. On a 16 MB card (alauda_card_ids id 0x73) num_zones is 1 and
uzonesize is 1000, so lba 1000 to 1023 pass the bounds check and index
element 1 of a one-element array.
alauda_read_data() also calls alauda_ensure_map_for_zone() before it
compares lba against max_lba, so any larger lba is used as an index
before the check rejects it. alauda_write_data() already checks first.
Bound lba by num_zones * uzonesize on both paths and move the read
side's call below the check. alauda_transport() reports
num_zones * uzonesize * blocksize for READ_CAPACITY, so the new bound is
exactly the range the device advertises and no block becomes
unreachable.
sd never issues an lba past the reported capacity; an SG_IO READ_10 can.

alauda_ensure_map_for_zone() ignores the value alauda_read_map()
returns. On failure lba_to_pba[zone] and pba_to_lba[zone] are left NULL,
and both callers dereference them immediately:
pba = MEDIA_INFO(us).lba_to_pba[zone][lba_offset];
alauda_read_map() returns USB_STOR_TRANSPORT_ERROR when either kcalloc()
fails or an alauda_get_redu_data() transfer fails, so one failed bulk
transfer is enough to reach the NULL dereference.
Return that value from alauda_ensure_map_for_zone() and check it in
alauda_read_data() and alauda_write_lba().

alauda_check_status2(), alauda_get_redu_data(), alauda_erase_block(),
alauda_read_block_raw() and alauda_write_block() build their commands in
stack arrays and pass them to usb_stor_bulk_transfer_buf().
alauda_check_status2() and alauda_erase_block() also read the reply back
into a stack array.
usb_stor_msg_common() sets URB_NO_TRANSFER_DMA_MAP only when the buffer
is us->iobuf. Every other buffer reaches usb_hcd_map_urb_for_dma(),
which has warned and returned -EAGAIN for a buffer on the stack since
commit 4568136620c6 ("usb: core: Check URB setup_packet and
transfer_buffer sanity"). On a host controller that uses DMA, all of
these commands fail.
Use us->iobuf, as alauda_reset_media() in this driver and the other
usb-storage subdrivers already do. The command is sent before the reply
is read, so both can use it; sddr09_read_status() does the same.

sddr09_read_map() builds the LBA <-> PBA translation tables while
servicing READ_CAPACITY. The logical block address assigned to each
physical block is decoded from device-controlled redundancy data:
lba = short_pack(ptr[7], ptr[6]); /* 16-bit device value */
lba = (lba & 0x07FF) >> 1; /* 0..1023 */
if (lba >= 1000)
goto possibly_erase;
lba += 1000*(i/0x400);
if (info->lba_to_pba[lba] != UNDEF) /* heap OOB read */
...
info->pba_to_lba[i] = lba;
info->lba_to_pba[lba] = i; /* heap OOB write */
The tables are allocated with numblocks entries each, where numblocks
is derived from the device-reported NAND chip geometry. For the
smallest chip in nand_flash_ids[] (1 MB: chipshift=20, pageshift=8,
blockshift=4):
numblocks = (1 << 20) >> (8 + 4) = 256
so info->lba_to_pba[] only has indexes 0..255, while a hostile device
may report any LBA up to 999 - the "lba >= 1000" check is the only
limit on the value, and there is no check that lba < numblocks. On a
1 MB card indexes 256..999 (744 of them) index the table out of
bounds, up to ~3 KB (744 * 4 bytes) past the end of the allocation.
Attack chain (malicious USB storage device):
1. An attacker presents a USB Mass Storage device spoofing a unit
matched in sddr09_usb_ids, e.g. 0x04e6:0x0003 (SanDisk
ImageMate SDDR09) or 0x0781:0x0200, on the victim USB bus; the
device is enumerated as a normal storage device.
2. ums-sddr09 binds (sddr09_probe -> us->transport =
sddr09_transport) and the SCSI layer issues READ_CAPACITY, which
is handled via sddr09_get_cardinfo() (chip geometry from the
device ID, choosing numblocks) and sddr09_read_map().
3. The device reports a 1 MB chip (numblocks = 256) and fills the
per-block redundancy data with LBA values in the 256..999 range,
driving info->lba_to_pba[lba] and info->pba_to_lba[i] accesses
out of bounds: a heap OOB read used in the map-building
conditionals plus a heap OOB write of the loop index i (the
physical block number, 0..255) at an offset the attacker controls
via the reported LBA, corrupting adjacent heap memory.
The device-controlled inputs, the missing bound check and the OOB
indexing are confirmed by end-to-end reproduction with a FunctionFS
based malicious device emulator; the driver logged out-of-bounds
indexes such as:
sddr09: LBA 256 seen for PBA -858993460 and 201
sddr09: LBA 258 seen for PBA 4513 and 203
Add the missing bounds check: since lba is unsigned it can only be
too large, so bail out with "lba >= numblocks" and mark the physical
block UNUSABLE instead of indexing the tables. This mirrors the
max_lba bounds checks already applied to the SCSI-address-derived LBA
in sddr09_read_data()/sddr09_write_data() and in the related sddr55
and alauda drivers.

Back in the 2.6.xx days, it was possible to for low-level drivers to set
the shost lock pointer. That is why there is a default lock and a
pointer to the shost lock.
However, support for this has long been removed, so drop
Scsi_Host.default_lock and make Scsi_Host.host_lock as the actual lock.
Getting the address of embedded host_lock structure just requires adding
a fixed offset value to the shost pointer. However, getting the value of
the host_lock pointer requires loading from a fixed offset to the shost
pointer. The latter should be very slightly slower, which is relevant as
this lock is used a lot throughout the core code and drivers.

realtek_cr_destructor() calls timer_delete() before the chip containing
the timer is freed. The timer callback may still be running and can
rearm itself, resulting in a use-after-free.
Use timer_shutdown_sync() to wait for the callback and prevent further
rearming. Do this unconditionally because ss_en may be changed after
the timer is armed.
Move timer_setup() into init_realtek_cr() so the timer is initialized
before any failure path can invoke the destructor.

ene_ub6250_probe() calls usb_stor_probe2(), which starts the usb-storage
infrastructure and schedules the delayed scan work. The driver then
calls ene_get_card_type(), which sends an ENE command through
ene_send_scsi_cmd() and the usb-storage bulk transfer helpers.
Both the delayed scan work, through usb_stor_Bulk_max_lun(), and
ene_get_card_type() use us->current_urb. The scan work serializes this
access with us->dev_mutex, but the ENE card-type probe does not. If the
scan work runs while ene_get_card_type() is still using us->current_urb,
usb_submit_urb() warns that the URB is already active.
Serialize ene_get_card_type() with us->dev_mutex, matching the locking
used by the scan path.

Commit 1892bf90677a ("USB: usb-storage: Fix use of bitfields for
hardware data in ene_ub6250.c") converted the media status fields from
bitfields to bit masks.
The original ene_transport() test called ene_init() only when neither
media type was ready:
!(sd_ready || ms_ready)
The converted test became:
!sd_ready || ms_ready
This is not equivalent. Restore the original semantics by testing that
both ready bits are clear before calling ene_init().

usb_stor_adjust_quirks() parses the usb-storage.quirks module
parameter into a new flag set and then applies it with the quirk
mask to override built-in flags.
The mask is meant to cover the flags that can be overridden by
the module parameter. The 'k' quirk character sets US_FL_NO_SAME,
but US_FL_NO_SAME is not included in the mask.
As a result, the module parameter can set US_FL_NO_SAME, but it
cannot clear a built-in US_FL_NO_SAME flag by providing an override
entry that omits 'k'.
Add US_FL_NO_SAME to the mask so that the module parameter can
override it in the same way as the other supported flags.

The PNY Elite Portable SSD (USB ID 154b:f009) is a sibling of the
already-quirked PNY Pro Elite SSDs (154b:f00b and 154b:f00d). Like its

firmware bugs when bound to the uas driver.
Without quirks, the device fails to complete READ CAPACITY commands
when accessed over UAS on a SuperSpeed (USB 3) port. The device
enumerates and reports as a SCSI direct-access device, but reports
zero logical blocks and never finishes spin-up:
usb 2-3: new SuperSpeed USB device number 8 using xhci_hcd
usb 2-3: New USB device found, idVendor=154b, idProduct=f009
usb 2-3: Product: PNY ELITE PSSD

scsi host0: uas
scsi 0:0:0:0: Direct-Access PNY PNY ELITE PSSD 0
sd 0:0:0:0: [sda] Spinning up disk...
[...10+ seconds of polling, no progress...]

sd 0:0:0:0: [sda] Read Capacity(10) failed: hostbyte=DID_ERROR
sd 0:0:0:0: [sda] 0 512-byte logical blocks: (0 B/0 B)
Tested each individual quirk to find the minimum that fixes this:
- US_FL_NO_ATA_1X alone: device hangs on spin-up
- US_FL_NO_REPORT_OPCODES alone: works on USB 2.0, hangs on USB 3.0
- US_FL_NO_ATA_1X | US_FL_NO_REPORT_OPCODES: works on both
With both quirks the device enumerates correctly while still using
the uas driver, and delivers full UAS throughput (~281 MB/s
sequential read on a USB 3.0 Gen 1 port).
The existing PNY Pro Elite entries (f00b, f00d) only set NO_ATA_1X,
but this device additionally chokes on REPORT OPCODES under
SuperSpeed.

Expands range of matched bcdDevice values for the VL817 quirk entry.
This is based on experience with Axagon EE35-GTR rev1 3.5" HDD

A UAS device can in principle contain multiple busses.
A reset on the USB level will reset them all. We
cannot reset a single bus.
In practical terms this does not matter, as only
one method of reset is implemented, but we should
not lie.

This was done entirely with mindless brute force, using
git grep -l '\<k[vmz]*alloc_objs*(.*, GFP_KERNEL)' |
xargs sed -i 's/\(alloc_objs*(.*\), GFP_KERNEL)/\1)/'
to convert the new alloc_obj() users that had a simple GFP_KERNEL
argument to just drop that argument.
Note that due to the extreme simplicity of the scripting, any slightly
more complex cases spread over multiple lines would not be triggered:
they definitely exist, but this covers the vast bulk of the cases, and
the resulting diff is also then easier to check automatically.
For the same reason the 'flex' versions will be done as a separate
conversion.

This is the result of running the Coccinelle script from
scripts/coccinelle/api/kmalloc_objs.cocci. The script is designed to
avoid scalar types (which need careful case-by-case checking), and
instead replace kmalloc-family calls that allocate struct or union
object instances:
Single allocations: kmalloc(sizeof(TYPE), ...)
are replaced with: kmalloc_obj(TYPE, ...)
Array allocations: kmalloc_array(COUNT, sizeof(TYPE), ...)
are replaced with: kmalloc_objs(TYPE, COUNT, ...)
Flex array allocations: kmalloc(struct_size(PTR, FAM, COUNT), ...)
are replaced with: kmalloc_flex(*PTR, FAM, COUNT, ...)
(where TYPE may also be *VAR)
The resulting allocations no longer return "void *", instead returning
"TYPE *".

In clang version 21.1 and later the -Wimplicit-enum-enum-cast warning
option has been introduced. This warning is enabled by default and can
be used to catch .queuecommand() implementations that return another
value than 0 or one of the SCSI_MLQUEUE_* constants. Hence this patch
that changes the return type of the .queuecommand() implementations from
'int' into 'enum scsi_qc_status'. No functionality has been changed.

min_t(unsigned int, a, b) casts an 'unsigned long' to 'unsigned int'.
Use min(a, b) instead as it promotes any 'unsigned int' to 'unsigned long'
and so cannot discard significant bits.
In this case the 'unsigned long' value is small enough that the result
is ok.

ongoing data transfer
When a UAS device is unplugged during data transfer, there is
a probability of a system panic occurring. The root cause is
an access to an invalid memory address during URB callback handling.
Specifically, this happens when the dma_direct_unmap_sg() function
is called within the usb_hcd_unmap_urb_for_dma() interface, but the
sg->dma_address field is 0 and the sg data structure has already been
freed.
The SCSI driver sends transfer commands by invoking uas_queuecommand_lck()
in uas.c, using the uas_submit_urbs() function to submit requests to USB.
Within the uas_submit_urbs() implementation, three URBs (sense_urb,
data_urb, and cmd_urb) are sequentially submitted. Device removal may
occur at any point during uas_submit_urbs execution, which may result
in URB submission failure. However, some URBs might have been successfully
submitted before the failure, and uas_submit_urbs will return the -ENODEV
error code in this case. The current error handling directly calls
scsi_done(). In the SCSI driver, this eventually triggers scsi_complete()
to invoke scsi_end_request() for releasing the sgtable. The successfully
submitted URBs, when being unlinked to giveback, call
usb_hcd_unmap_urb_for_dma() in hcd.c, leading to exceptions during sg
unmapping operations since the sg data structure has already been freed.
This patch modifies the error condition check in the uas_submit_urbs()
function. When a UAS device is removed but one or more URBs have already
been successfully submitted to USB, it avoids immediately invoking
scsi_done() and save the cmnd to devinfo->cmnd array. If the successfully
submitted URBs is completed before devinfo->resetting being set, then
the scsi_done() function will be called within uas_try_complete() after
all pending URB operations are finalized. Otherwise, the scsi_done()
function will be called within uas_zap_pending(), which is executed after
usb_kill_anchored_urbs().
The error handling only takes effect when uas_queuecommand_lck() calls
uas_submit_urbs() and returns the error value -ENODEV . In this case,
the device is disconnected, and the flow proceeds to uas_disconnect(),
where uas_zap_pending() is invoked to call uas_try_complete().

Discovered by Atuin - Automated Vulnerability Discovery Engine.
new_pba comes from the status packet returned after each write.
A bogus device could report values beyond the block count derived
from info->capacity, letting the driver walk off the end of
pba_to_lba[] and corrupt heap memory.
Reject PBAs that exceed the computed block count and fail the
transfer so we avoid touching out-of-range mapping entries.

Currently if a user enqueues a work item using schedule_delayed_work() the
used wq is "system_wq" (per-cpu wq) while queue_delayed_work() use
WORK_CPU_UNBOUND (used when a cpu is not specified). The same applies to
schedule_work() that is using system_wq and queue_work(), that makes use
again of WORK_CPU_UNBOUND.
This lack of consistency cannot be addressed without refactoring the API.
alloc_workqueue() treats all queues as per-CPU by default, while unbound
workqueues must opt-in via WQ_UNBOUND.
This default is suboptimal: most workloads benefit from unbound queues,
allowing the scheduler to place worker threads where they’re needed and
reducing noise when CPUs are isolated.
This continues the effort to refactor workqueue APIs, which began with
the introduction of new workqueues and a new alloc_workqueue flag in:
commit 128ea9f6ccfb ("workqueue: Add system_percpu_wq and system_dfl_wq")
commit 930c2ea566af ("workqueue: Add new WQ_PERCPU flag")
This change adds a new WQ_PERCPU flag to explicitly request
alloc_workqueue() to be per-cpu when WQ_UNBOUND has not been specified.
With the introduction of the WQ_PERCPU flag (equivalent to !WQ_UNBOUND),
any alloc_workqueue() caller that doesn’t explicitly specify WQ_UNBOUND
must now use WQ_PERCPU.
Once migration is complete, WQ_UNBOUND can be removed and unbound will
become the implicit default.

A kernel memory leak was identified by the 'ioctl_sg01' test from Linux
Test Project (LTP). The following bytes were mainly observed: 0x53425355.
When USB storage devices incorrectly skip the data phase with status data,
the code extracts/validates the CSW from the sg buffer, but fails to clear
it afterwards. This leaves status protocol data in srb's transfer buffer,
such as the US_BULK_CS_SIGN 'USBS' signature observed here. Thus, this can
lead to USB protocols leaks to user space through SCSI generic (/dev/sg*)
interfaces, such as the one seen here when the LTP test requested 512 KiB.
Fix the leak by zeroing the CSW data in srb's transfer buffer immediately
after the validation of devices that skip data phase.
Note: Differently from CVE-2018-1000204, which fixed a big leak by zero-
ing pages at allocation time, this leak occurs after allocation, when USB
protocol data is written to already-allocated sg pages.

git://git.kernel.org/pub/scm/linux/kernel/git/gregkh/usb
Pull USB / Thunderbolt updates from Greg KH:
"Here is the big set of USB and Thunderbolt changes for 6.16-rc1.
Included in here are the following:
- USB offload support for audio devices.
I think this takes the record for the most number of patch series
(30+) over the longest period of time (2+ years) to get merged
properly.
Many props go to Wesley Cheng for seeing this effort through, they
took a major out-of-tree hacked-up-monstrosity that was created by
multiple vendors for their specific devices, got it all merged into
a semi-coherent set of changes, and got all of the different major
subsystems to agree on how this should be implemented both with
changes to their code as well as userspace apis, AND wrangled the
hardware companies into agreeing to go forward with this, despite
making them all redo work they had already done in their private
device trees.
This feature offers major power savings on embedded devices where a
USB audio stream can continue to flow while the rest of the system
is sleeping, something that devices running on battery power really
care about. There are still some more small tweaks left to be done
here, and those patches are still out for review and arguing among
the different hardware companies, but this is a major step forward
and a great example of how to do upstream development well.
- small number of thunderbolt fixes and updates, things seem to be
slowing down here (famous last words...)
- xhci refactors and reworking to try to handle some rough corner
cases in some hardware implementations where things don't always
work properly
- typec driver updates
- USB3 power management reworking and updates
- Removal of some old and orphaned UDC gadget drivers that had not
been used in a very long time, dropping over 11 thousand lines from
the tree, always a nice thing, making up for the 12k lines added
for the USB offload feature.
- lots of little updates and fixes in different drivers
All of these have been in linux-next for over 2 weeks, the USB offload
logic has been in there for 8 weeks now, with no reported issues"
