The "nop" driver isn't a do-nothing-stub but supports a couple functions
like clock on/off or is able to use a voltage regulator. This patch
simply renames the driver to "generic" since it is easy possible to
extend it by a simple function istead of writing a complete driver.

Add clk_rate parameter to platform data. If supplied, the
NOP phy driver will program the clock to that rate during probe.
Also add 2 flags, needs_vcc and needs_reset.
If the flag is set and the regulator couldn't be found
then the driver will bail out with -EPROBE_DEFER.

We add a platform_data to pass the desired mode
for the nop-xceiv so it can work on USB2 and USB3
modes.

nop-usb-xceiv was polluting otg.h with its own
function prototypes. Move those prototypes to
a nop-usb-xceiv.h header.
