In some device configurations there's no radio or radio support in the
driver. That's OK, as the driver sets itself up accordingly. However
on tear-down in these caes it's still trying to tear down radio
related context when there isn't anything there, leading to
dereferences through a null pointer and chaos follows.
How this bug survived unfixed for 11 years in the pvrusb2 driver is a
mystery to me.

On 32-bits archs, a signed integer has 31 bits plus on extra
bit for signal. Due to that, touching the 32th bit with something
like:
int bar = 1 << 31;
has an undefined behavior in C on 32 bit architectures, as it
touches the signal bit. This is warned by cppcheck.
Instead, force the numbers to be unsigned, in order to solve this
issue.

Instead of filling in the struct v4l2_capability device_caps
field, fill in the struct video_device device_caps field.
That way the V4L2 core knows what the capabilities of the
video device are.
But this only really works if all drivers use this, so convert
all usb drivers in this patch.

The strncpy() function is being deprecated upstream. Replace
it by the safer strscpy().
While here, replace a few occurences of strlcpy() that were
recently added to also use strscpy().

This driver is the only V4L driver that does not set unlocked_ioctl
to video_ioctl2.
The only thing that pvr2_v4l2_ioctl does besides calling video_ioctl2
is calling pvr2_hdw_commit_ctl(). Add pvr2_hdw_commit_ctl() calls to
the various ioctls that need this, and we can replace pvr2_v4l2_ioctl
by video_ioctl2.

NOTE! On almost all architectures, the EPOLL* constants have the same
values as the POLL* constants do. But they keyword here is "almost".
For various bad reasons they aren't the same, and epoll() doesn't
actually work quite correctly in some cases due to this on Sparc et al.
The next patch from Al will sort out the final differences, and we
should be all done.

The pvrusb2 code appears to have a some old workaround code for xawtv that causes a
WARN() due to an unrecognized pixelformat 0 in v4l2_ioctl.c.
Since all other MPEG drivers fill this in correctly, it is a safe assumption that
this particular problem no longer exists.
While I'm at it, clean up the code a bit.

Using enums makes easier to document, as it can use kernel-doc
markups. It also allows cross-referencing, with increases the
kAPI readability.
Please notice that now cx88_querycap() has to have a default for
the VFL type, as there are more types than supported by the driver.

Due to the 80-cols restrictions, and latter due to checkpatch
warnings, several strings were broken into multiple lines. This
is not considered a good practice anymore, as it makes harder
to grep for strings at the source code.
As we're right now fixing other drivers due to KERN_CONT, we need
to be able to identify what printk strings don't end with a "\n".
It is a way easier to detect those if we don't break long lines.
So, join those continuation lines.
The patch was generated via the script below, and manually
adjusted if needed.

The VIDIOC_QUERYCAP function should set device_caps, but this was missing.
In addition, it set the version field as well, but that should be done by
the core, not by the driver.
If a driver doesn't set device_caps the v4l2 core will issue a WARN_ON, so
it's important that this is set correctly.

The issue was discovered by static analysis. It turns out that code is
somewhat insane, being
if (x) {...} else { if (x) {...} }
Edited it to do the only reasonable thing, which is to log the
information about the failed call. The most descriptive logging commands
set is taken from original code.

This kind of memcpy() is error-prone. Its replacement with a struct
assignment is prefered because it's type-safe and much easier to read.
Found by coccinelle. Hand patched and reviewed.

Write-only ioctls should have a const argument in the ioctl op.
Do this conversion for vidioc_s_crop.
Adding const for write-only ioctls was decided during the 2012 Media Workshop.
