range.h works with struct range data type. The resource_size_t
is an alien here.

Replace #include <asm/export.h> with #include <linux/export.h>.

DWC3 as implemented in Cavium SoC is using UCTL bridge unit
between I/O interconnect and USB controller.
Currently there is no bond with dwc3 core code, so if anything goes
wrong in UCTL setup dwc3 is left in reset, which leads to bus error
while trying to read any device register. Thus any failure in UCTL
initialization ends with kernel panic.
To avoid this move Octeon DWC3 glue code from arch/mips and make it
proper glue driver which is used instead of dwc3-of-simple.

A couple more clean-ups in the MIPS code.
The DT of_device.h and of_platform.h date back to the separate
of_platform_bus_type before it was merged into the regular platform bus.
As part of that merge prepping Arm DT support 13 years ago, they
"temporarily" include each other. They also include platform_device.h
and of.h. As a result, there's a pretty much random mix of those include
files used throughout the tree. In order to detangle these headers and
replace the implicit includes with struct declarations, users need to
explicitly include the correct includes.

Move all register access code into separate functions and
provide their no-op version for non Octeon platforms.
Later it might be possible to replace them with standard
Linux functions, however datasheets are not publicly available
and I have only one Octeon board to test, so lets stay on safe
side for now.

Switch to the CPU hotplug core state tracking and synchronization
mechanim. This unfortunately requires to add dead reporting to the non CPS
platforms as CPS is the only user, but it allows an overall consolidation
of this functionality.
No functional change intended.

Fundamentally semaphores are a counted primitive, but
DEFINE_SEMAPHORE() does not expose this and explicitly creates a
binary semaphore.
Change DEFINE_SEMAPHORE() to take a number argument and use that in the
few places that open-coded it using __SEMAPHORE_INITIALIZER().

Console output currently looks like USB clocks initialized succesfully
even in case of error. Fix that and use consistently dev_err for fatal
errors otherwise dev_warn.

Some of them were pointless because CONFIG_CAVIUM_RESERVE32 is now always
defined, some were not enough (Yu Zhao reported
"Failed to allocate CAVIUM_RESERVE32 memory area" error).
Removing the directives allows for compiler coverage of RESERVE32 code and
replacing one of [always-true] "ifdef" with a compiler conditional fixes
the [cosmetic] error message.

This options is used to reserve a shared memory region for user processes
to use for hardware memory buffers. The actual code to support the option
comes in the following patch.

Now that the irq_data_update_affinity helper exists, enforce its use
by returning a a const cpumask from irq_data_get_affinity_mask.
Since the previous commit already updated places that needed to call
irq_data_update_affinity, this commit updates the remaining code that
either did not modify the cpumask or immediately passed the modified
mask to irq_set_affinity.

We should call of_node_put() for the reference 'uctl_node' returned by
of_get_parent() which will increase the refcount. Otherwise, there will
be a refcount leak bug.

Some CN66XX series Octeon II chips seem to hang if a reset is issued on
XAUI initialization. Avoid the hang by disabling the reset.
Tested on SNIC10E.

Pass a boolean flag to indicate if swiotlb needs to be enabled based on
the addressing needs, and replace the verbose argument with a set of
flags, including one to force enable bounce buffering.
Note that this patch removes the possibility to force xen-swiotlb use
with the swiotlb=force parameter on the command line on x86 (arm and
arm64 never supported that), but this interface will be restored shortly.

The only usage of these structs is to pass their address to either
irq_domain_add_tree() or irq_domain_create_linear(), both which takes
pointers to const struct irq_domain_ops. Make them const to allow the
compiler to put them in read-only memory.
