Implement device supported verb APIs for shared receive queue.
SRQ is supported for userspace only. ionic_create_srq() returns
-EOPNOTSUPP when there is no ucontext, so kernel ULPs cannot create an
SRQ.

In preparation for SRQ support, segregate rq related fields from ionic_qp
into a new ionic_rq struct. This refactoring enables both non-SRQ and
future SRQ code to leverage the common rq init/destroy infrastructure
around ionic_qp_rq_init/_destroy with no functional changes.

When the LIF advertises qid allocation for CQ, let firmware return
the completion queue ID instead of allocating it in the driver bitmap.
Split CQ create/destroy into distinct phases so cq_tbl is updated only
after the ID is known and removed before the destroy admin command
returns the ID to firmware, avoiding a race where a concurrent create
reusing the same cqid could have its xarray entry erased by a stale
destroy.

Commit 7e53b31acc7f ("RDMA/core: Create and destroy rdma_counter using
rdma_zalloc_drv_obj()") requires drivers implementing counter ops to
embed struct rdma_counter in a driver-specific struct, register its size
via INIT_RDMA_OBJ_SIZE, and provide a counter_init callback.
The ionic driver was merged without this adaptation, causing a NULL
pointer dereference in alloc_and_bind() since rdma_zalloc_drv_obj()
allocates zero bytes when size_rdma_counter is unset.
Consolidate struct ionic_counter into a new struct ionic_rdma_counter
that embeds struct rdma_counter, replace the xarray with a lightweight
ida for ID allocation, and add the required counter_init and
INIT_RDMA_OBJ_SIZE declarations.

Enable user space applications to access the PHC state page when
firmware RDMA completion timestamp is supported.
This mapping allows user space to convert RDMA completion timestamps
to system wall time without kernel transitions, minimizing latency
overhead. Applications can directly read the PHC state through mmap,
enabling efficient timestamp correlation for precision timing
applications.

The DMA iterator logic was mixed into verbs and umem-specific code,
forcing all users to include rdma/ib_umem.h. Move the block iterator
logic into iter.c and rdma/iter.h so that rdma/ib_umem.h and
rdma/ib_verbs.h can be separated in a follow-up patch.
