parisc: move definition of PAGE0 to asm/page.h

This was defined in asm/pdc.h which needs to include asm/page.h for __PAGE_OFFSET. This leads to an include loop so that page.h eventually will include pdc.h again. While this is no problem because of header guards, it is a problem because some symbols may be undefined. There are numerous broken references to Documentation files (in other Documentation files, in comments, etc.). These broken references are caused by typo's in the references, and by renames or removals of the Documentation files. Some broken references are simply odd. mm: replace various uses of num_physpages by totalram_pages

Sizing of memory allocations shouldn't depend on the number of physical pages found in a system, as that generally includes (perhaps a huge amount of) non-RAM pages. The amount of what actually is usable as storage should instead be used as a basis here. Some of the calculations (i.e. those not intending to use high memory) should likely even use (totalram_pages - totalhigh_pages). parisc: remove driver_data direct access of struct device

In the near future, the driver core is going to not allow direct access to the driver_data pointer in struct device. Instead, the functions dev_get_drvdata() and dev_set_drvdata() should be used. These functions have been around since the beginning, so are backwards compatible with all older kernel versions. Move DMA-mapping.txt to Documentation/PCI/. DMA-mapping.txt was supposed to be moved from Documentation/ to Documentation/PCI/. The 00-INDEX files in those two directories were updated, along with a few other text files, but the file itself somehow escaped being moved, so move it and update more text files and source files with its new location.
