Instead of using the out-of-line EWMA calculation, use DECLARE_EWMA() to create static inlines. On x86/64 this results in code that's one byte larger (for me), but reduces struct link_ant and struct link size by the two unsigned long values that store the parameters each. rt2x00: fix rfkill regression on rt2500pci

As reported by Niels, starting rfkill polling during device probe (commit e2bc7c5, generally sane change) broke rfkill on rt2500pci device. I considered that bug as some initalization issue, which should be fixed on rt2500pci specific code. But after several attempts (see bug report for details) we fail to find working solution. Hence I decided to revert to old behaviour on rt2500pci to fix regression. Additionally patch also unregister rfkill on device remove instead of ifconfig down, what was another issue introduced by bad commit. mac80211: add vif to flush call

rt2x00: Fix FSF address in file headers

Several files refer to an old address for the Free Software Foundation in the file header comment. Resolve by replacing the address with the URL so that we do not have to keep updating the header comments anytime the address changes. rt2x00: add rt2x00_has_cap_* helpers

The rt2x00 code directly accesses the 'cap_flags' field of 'struct rt2x00_dev' when checking presence of a given capability flag. The direct access needs long expressions which lowers readability of the code. Add a few helper functions which can be used to test device capabilities without directly accessing the cap_flags filed. rt2x00: rt2800lib: add default_power3 field for three-chain devices

The actual code uses two default TX power values. This is enough for 1T and for 2T devices however on 3T devices another value is needed for the third chain. Add a new field to struct channel_info and initialize it from the 'rt2800_probe_hw_mode' function. Also modify the 'rt2800_config_channel' to handle the new field as well.
