Several fixes as well where the +1 was missing.

Done via coccinelle scripts like:

@@
struct resource *ptr;
@@

- ptr->end - ptr->start + 1
+ resource_size(ptr)

and some grep and typing.

Mostly uncompiled, no cross-compilers.

Add appropriate <linux/prefetch.h> include for prefetch users

After discovering that wide use of prefetch on modern CPUs
could be a net loss instead of a win, net drivers which were
relying on the implicit inclusion of prefetch.h via the list
headers showed up in the resulting cleanup fallout. Give
them an explicit include via the following $0.02 script.

=========================================
#!/bin/bash
MANUAL=""
for i in `git grep -l 'prefetch(.*)' .` ; do
 grep -q '<linux/prefetch.h>' $i
 if [ $? = 0 ] ; then
 continue
 fi

 (	echo '?^#include <linux/?a'
 echo '#include <linux/prefetch.h>'
 echo .
 echo w
 echo q
 ) | ed -s $i > /dev/null 2>&1
 if [ $? != 0 ]; then
 echo $i needs manual fixup
 MANUAL="$i $MANUAL"
 fi
done
echo ------------------- 8\<----------------------
echo vi $MANUAL

drivers/net: avoid some skb->ip_summed initializations

fresh skbs have ip_summed set to CHECKSUM_NONE (0)

We can avoid setting again skb->ip_summed to CHECKSUM_NONE in drivers.

Introduce skb_checksum_none_assert() helper so that we keep this
assertion documented in driver sources.

Change most occurrences of :

skb->ip_summed = CHECKSUM_NONE;

by :

skb_checksum_none_assert(skb);

net: preserve ifreq parameter when calling generic phy_mii_ioctl().

The phy_mii_ioctl() function unnecessarily throws away the original ifreq.
We need access to the ifreq in order to support PHYs that can perform
hardware time stamping.

Two maverick drivers filter the ioctl commands passed to phy_mii_ioctl().
This is unnecessary since phylib will check the command in any case.

Now that core network takes care of trans_start updates, dont do it
in drivers themselves, if possible. Drivers can avoid one cache miss
(on dev->trans_start) in their start_xmit() handler.

Exceptions are NETIF_F_LLTX drivers
