mISDN: array underflow in open_bchannel()

isdn source code uses a not-current coding style. Update the coding style used on a per-line basis so that git diff -w shows only elided blank lines at EOF. Fix for a -Warray-bounds warning. mixer_notify_update() tries to write to ((CAPI_MSG *) msg)->info.facility_req.structs[3] while structs is defined as byte structs[1]. module_param: make bool parameters really bool (drivers & misc)

module_param(bool) used to counter-intuitively take an int. In fddd5201 (mid-2009) we allowed bool or int/unsigned int using a messy trick. It's time to remove the int/unsigned int option. USB: convert some miscellanies drivers to use module_usb_driver()

This converts the remaining USB drivers in the kernel to use the module_usb_driver() macro which makes the code smaller and a bit simpler. Added bonus is that it removes some unneeded kernel log messages about drivers loading and/or unloading.
