This is in preparation of splitting the current monolithic driver up and add support for the upcoming NVMe over Fabrics standard. The drivers/nvme/host/ is chose to leave space for a NVMe target implementation in addition to this host side driver. This both simplifies the code a lot and allow for a better structure. For example now the LighNVM code can construct commands without knowing the details of the underlying I/O descriptors. Or a future NVMe over network target could inject commands, as well as could the SCSI translation and ioctl code be reused for such a beast. Fix this up by removing one of them, and centralizing the generation of the other positive values in a single place. Also fix up a few places that didn't handle the NVMe error codes properly.
